Does Sunscreen Cause Skin Cancer? Of course not.
Melanoma diagnoses have climbed for fifty years. Sunscreen use climbed alongside them. Put those two lines on the same chart and it is easy to draw a conclusion that the data does not support.
Sunscreen does not cause skin cancer. The rise in diagnoses has a well documented explanation, and it has more to do with how we look for melanoma than with what people put on their skin.
The most important thing on this page: wearing sunscreen beats not wearing sunscreen, every time. Whatever you take from the rest of this, do not let a myth talk you out of protection. The filter debate is a question about which sunscreen. It was never a question about whether.
The number that breaks the theory
If sunscreen were causing melanoma, deaths from melanoma would be rising with the diagnoses. They are not.
Since the 1970s melanoma incidence has risen sharply while melanoma-specific mortality has stayed roughly flat. That gap is the whole story. A disease that is genuinely becoming more common and more dangerous kills more people. Melanoma is being found far more often without killing proportionally more people, which points at detection rather than at cause.
What changed: we started looking
Several things happened at once, and all of them increase the number of melanomas found without increasing the number that exist.
- Screening became routine. Full body skin checks went from unusual to standard practice.
- The biopsy threshold dropped. With rising awareness, clinicians began removing lesions they would once have watched. More biopsies means more findings.
- Diagnostic criteria widened. Lesions classified as benign decades ago are now classified as melanoma in situ.
- The population aged. Melanoma risk climbs with age, and the age distribution shifted.
The in situ numbers show this clearly. In the US, the ratio of melanoma in situ to invasive melanoma went from 0.14 in 1982 to 0.93 in 2018. In situ melanoma is confined to the top layer of skin. Finding vastly more of it, while deaths hold steady, is what detection driven growth looks like.
Researchers have a word for this. Estimates of melanoma overdiagnosis, meaning diagnoses that would never have caused harm in a person's lifetime, range from 29% to 60% depending on the analysis. One ecological study estimated that the large majority of in situ diagnoses in 2018 were attributable to overdiagnosis.
None of that means melanoma is not serious. It is. It means the diagnosis curve is measuring our detection effort as much as it is measuring disease.
Where the sunscreen suspicion comes from
There is a real finding underneath the myth, and it is worth stating accurately rather than dismissing.
In 2019 and 2020, FDA studies found that several chemical UV filters are absorbed through skin into the bloodstream at concentrations above the threshold that would normally trigger further toxicology testing. Oxybenzone, avobenzone, octocrylene, homosalate, octisalate and octinoxate were among them.
Absorption is not harm. No study has shown these filters cause cancer. But the FDA has said it needs more safety data, and of the 16 filters it reviewed, only two were classified as generally recognized as safe and effective: zinc oxide and titanium dioxide. The other twelve were not declared unsafe. They were declared insufficiently studied, and that data has not arrived.
That gap between "we do not have enough data" and "it is dangerous" is where the sunscreen causes cancer claim grows. The honest position sits in between: some filters are absorbed, the safety questions are open, and open questions are not the same as evidence of harm.
What sunscreen is established to do
The FDA sunscreen monograph permits one claim about skin cancer, and the wording is precise for a reason. A broad spectrum sunscreen of SPF 15 or higher, if used as directed with other sun protection measures, decreases the risk of skin cancer and early skin aging caused by the sun.
Read that carefully. Not prevents. Decreases the risk. And only alongside the rest of it: shade, timing, clothing, reapplication. Sunscreen is one input, not a shield.
The practical version
If the absorption data is what worries you, the answer is not skipping sunscreen. It is choosing a filter that is not in question.
